You could make a costume like your in bewteen an X-ray machine- you know, like a frame in front and a frame in back with the front view and back view of a skeleton respectively painted on each side.
You could be a hosptila patient with a gown on and the flap in back open (if you're brave enough).
If you dress as a doctor- pick a specific type and be creative with accessories for like a podiatrist/gynecologist/ urologist/etc.

How about folding Geisha fans(cheap ones are available) or partial masks with the holder for the women and cheap Top Hats that can all be bought at the same place the bead were bought at.
